We’ve spent hundreds of hours on the phone with pharmacies, hospital discharge planners, and durable medical equipment providers, making sure a hospital discharge goes smoothly. We’ve been at the geriatric behavrioal hospital when untreated or mismanaged medication and

care plans result in avoidable disruptions. We’ve been at the ER to meet with a new resident who is tired of ending up there for UTIs that should have been managed elsewhere. We have a very unique perspective on senior care in America.

 

We’ve built & grown hospice agencies, assisted living facilities and personal care homes. We’ve run home care & home health agencies.
